Medicaid is now the single largest source of coverage in the US in terms of the number of individuals enrolled. With the continued federal efforts to reshape and restructure the program, states continue to explore the limits of innovation in order to improve value, reduce the rate of expenditure growth, and shape the program to meet their policy and political objectives. When these changes are perceived to have gone too far, some stakeholders have resorted to the courts with mixed success.
