This session will explore a case study of a large health organization that went “upstream” to address the social determinants of health where its patients live. You’ll engage with the speakers to discuss how the organization built the business case for intervening upstream, quantified impacts of these interventions, and optimized portfolios to meet the unique needs of its community. Small groups will also assess the ROI of various interventions, brainstorm current capabilities, and create real implementation options within your yourganization.
