When you get to Zero, and nothing to measure, then what?

The quest for zero accidents has resulted in an ever-decreasing rate of accidents in most organizations. Logically, at some point in the future, we will reach such low numbers of accidents that there will be no value in measuring them. Or, if you believe you will reach zero, what do you measure then? How will you the know whether you are ‘safe’, given that leading metrics (near miss, hazard reports) are notoriously unreliable and invalid? This presentation will discuss a new approach, called ‘latent and lurking indicators’, that measures ‘shaping factors’ at systemic and cultural levels. These indicators have now been introduced to several organizations, to provide an ‘around-the-corner’ view of safety, instead of ‘rear mirror’ views.
