In 2014, CommCare Corporation opened Riviere’ de Soleil, the first-of-a-kind household model of skilled nursing with a 130-bed community care residence in Mansura, LA. This culture change prototype vision initiated in 2011, continues to evolve in response to balancing the influences of regulatory, training, operational, and financial challenges, as the basis for four (4) new household model facilities in Louisiana designed by KTH Architects, Inc.

Culture Change is an iterative challenge requiring early foresight and continuous scrutiny for insight in skilled nursing and rehabilitative care. It is the Lessons Learned from the first facility that form a renewed strategic alignment of planning and design principles addressing new opportunities to enhance operational efficiency and facility performance. Key guiding principles thrive, including front doors and porches at each home, warming kitchens at the hearth, staff presence at the residence, resident beds in quiet clusters, neighborhood commons within reach, and inviting community amenities and open spaces at Trinity Trace Community Care Center in Covington, LA.
