AI-powered, Remote Wound Care: Delivering Profound Clinical Outcomes in Home Health (Room 263/264)

Wound care is a massive challenge in home health. While over 30% of patients in home health have a wound, providing standardized, best practice care is a challenge. In 2020, a home health agency that provides services across five states, launched a nurse-led, remote-enabled wound care program with their innovative technology partner. To underpin this new, virtual wound care program, the nursing team adopted an AI-powered, digital wound care platform to allow frontline clinicians to easily capture high precision wound images in the home and then share this data with the remote wound experts to augment clinical decision making to drive more preventive care. A benefits study of the program revealed significant improvement after only 6 months: - Reduction in 60-day rehospitalization rate. - Reduction in incidence of new wounds in home health. - Improvement in wound healing rate. - Reduction in documentation time. - Reduction in nursing visits per wound episode.
