Faced with declining payments (as well as heightened expectations from health plans and patients), healthcare delivery systems struggle with containing care costs while delivering consistently good outcomes for every patient. One West Coast health system has developed a systematic approach to delivering higher-value care by integrating cost and outcomes data, identifying drivers of variance and engaging clinical champions to lead practice change with their colleagues.
