The information on “near misses” reported through UCSD Medical Center’s web-based event-reporting system has proven very valuable to the center’s Ergonomics and Injury Prevention programs. Near misses include events, such as medication or surgical errors, that may result in patient harm, as well as equipment and facility issues that could result in injuries to employees or damage to equipment. Information from these reports facilitates remediation of unsafe working conditions and prevention of musculoskeletal injuries.
