Chief Warrant Officer 5 Tammy D. Surratt is a native of Petersburg, Nebraska. She entered the U.S. Army on May 4, 1992, attended Basic training at Fort Dix, New Jersey, and Advanced Individual Training at Fort Gordon, Georgia. Her assignments include: Software Analyst-Operator, 200th Quartermaster Detachment, 10th Mountain Division, Fort Drum, New York (1992-1996); Information Systems Operator Analyst, Computer Information Center, HHC, Aviation and Missile Command, Redstone Arsenal, Alabama, (1996-2001); Information Systems Technician, G-6, 1st Cavalry Division, Fort Hood, Texas, (2001-2007); Senior Information Systems Technician, G-6, Fires Center of Excellence, Fort Sill, Oklahoma (2008-2012); Senior Network Operations Technician, G-6, III Corps, Fort Hood (2012-2016). Chief Warrant Officer 5 Surratt deployed twice in support of Operation Iraqi Freedom (2004- 2005, 2006-2007), Operation Enduring Freedom (2013-2014), and Operation Inherent Resolve (2015-2016). Chief Warrant Officer 5 Surratt’s military education includes the Software Analyst Course (74F), Primary Leadership Development Course, Basic Noncommissioned Officers (74B), Warrant Officer Candidate School, Warrant Officer Basic Course (251A), Warrant Officer Advanced Course (255A), Warrant Officer Staff Course and the Warrant Officer Senior Staff Course. She has a Bachelor of Science degree in Information Systems Management from the University of Maryland University College. Her awards and decorations include: Bronze Star Medal with 2 Oak Leaf Clusters, Defense Meritorious Service Medal, Meritorious Service Medal with 1 Oak Leaf Cluster, Army Commendation Medal with 2 Oak Leaf Clusters, and Army Achievement Medal with 3 Oak Leaf Clusters.
